The Medicaid Service Coordinator acts as a liaison for children and adults with Developmental Disabilities by assisting them in accessing their desires and needs within their community. The Medicaid Service Coordinator facilitates assessment, linkage, and referral for the desired or needed service/support as it applies to each person. This is accomplished through coordination and service planning, advocacy, and monitoring of services.
